Gestational Surrogate Requirements
Women who want to become a surrogate must first meet basic surrogate requirements to move forward, and those requirements are designed to confirm that a surrogate candidate is physically, mentally, and emotionally prepared for the journey.
You might make a great surrogate if you meet these requirements:

Be between 21-38 years old

Have a BMI (body mass index) of 32 or below (calculate your BMI)

Have had at least one healthy, uncomplicated pregnancy and birth within the last 10 years

Be a U.S. citizen or legal permanent resident

Live a smoke-free lifestyle and avoid illegal drugs

Emotional, Mental, and Healthy Pregnancy Surrogate Qualifications
To make sure you feel safe, supported, and ready for this unique journey, we have wellness guidelines in place. These aren't meant to judge or exclude—they exist to protect you and the baby and to give you the best chance at a smooth, healthy pregnancy. As part of the screening process, gestational surrogates also need to show emotional readiness for the experience ahead.
Every journey begins with a conversation with a licensed mental health professional. This isn't a test, it's a psychological evaluation to assess mental health and emotional readiness, and an opportunity to talk openly about:

Your experiences with past pregnancies

Your current emotional well-being

Your hopes and reasons for wanting to be a surrogate

The support system you have in your life
Your emotional & physical safety is important to us. Certain medications or untreated conditions, such as depression, may need to be resolved before continuing, because your well-being always comes first.
The goal is to make sure you feel emotionally prepared and cared for, so that you have the strength and encouragement needed throughout the process. If challenges come up, we'll connect you with resources and support, including women who've gone through similar experiences and can be helpful during the surrogacy journey.
Think You Don’t Qualify?
If your qualifications don't meet the list above, you may still be eligible for our MAPLE Program, which offers compensation up to $50,000 to surrogates who meet more flexible requirements.

What Happens If I Don't Meet the Surrogate Requirements?
Sometimes, the timing just isn't right. Certain health or emotional factors may mean it's better to wait before beginning your journey. In some cases, parts of your medical history, current health, or mental health may affect when you can become a surrogate.
This isn't a rejection, it's a way of caring for you. With patience and support, many women who don't qualify right away are later able to move forward successfully. Eligibility can sometimes change after treatment, more time after pregnancy, or an updated medical review.
